INITIAL CONTACT via FIRST RESPONDER

Types of Calls for Service:
  • Overdoses/Intoxication
  • Welfare Checks
  • Suicidal/Mental Health related
  • Domestic Related

COAR TEAM ALERTED

First Responder flags the call for the COAR team

COAR FOLLOWS UP

COAR designates appropriate team member(s) who provides follow up within 24-72 hours via phone call or home visit:
  • Community Mental Health Clinician
  • Peer Recovery Navigator
  • Community Outreach Officer

WE CONNECT YOU

COAR connects with individual and/or family members and completes community resource screening and universal release of information

COMMUNITY SERVICES

COAR links individual and/or family to appropriate community services

FOLLOW UP

COAR continues to follow up with individual and/or family until links are successful and all barriers are worked through or until family ceases services
